Intervención psicológica en artritis reumatoide: caso clínico

Abstract
espanolSe presenta la intervencion psicologica llevada a cabo en el caso de un varon de 44 anos con Artritis Reumatoide. El motivo de consulta es un alto grado de tristeza, ansiedad e irritabilidad. Para la evaluacion se emplearon los siguientes instrumentos y tecnicas: entrevista clinica, autorregistros y pruebas de autoinforme (Cuestionario Biografico; Inventario de Situaciones y Respuestas de Ansiedad –ISRA–; Cuestionario Tridimensional para la Depresion –CTD–; Inventario de Expresion de Ira Estado-Rasgo –STAXI 2–; Cuestionario de Dolor de McGill e Inventario de Sugestionabilidad –IS–). La evaluacion permitio establecer el diagnostico, segun criterios DSM-IV-TR (APA, 2000), de «Trastorno por dolor asociado a factores psicologicos y a enfermedad medica, de caracter cronico». A partir del analisis funcional del caso se fijaron los objetivos terapeuticos y se planifico el tratamiento cognitivo-conductual, dirigido al control de las emociones negativas y de los niveles de dolor del paciente, asi como al manejo de su incapacidad. Los resultados ponen de manifiesto la eficacia de las tecnicas de tratamiento empleadas. EnglishThis a case of a 44 years old male with rheumatoid arthritis is presented. Assessment was made with the following instruments: clinical interview; repeated self- reports and Structured Interview, Inventory of Situations and Responses of Anxiety –ISRA–; Treedimensional Questionaire of Depression –CTD–; State-Trait Anger Expression Inventory 2 –STAXI 2–; The McGill Pain Questionnaire and Inventory of Suggestibility –IS–. Evaluation allowed us to conclude in a diagnosis of «Pain disorder associated with both psychological factors and a general medical condition», according to DSM-IV-TR criteria (APA, 2000). After the functional analysis of the problem, we established the therapeutic aims. Cognitive-behavioral treatment was carried out in order to control negative emotions and pain levels. Results showed that the treatment techniques used are effective to reduce negative emotions and pain perception levels.
