Ziel dieser retrospektiven Untersuchung war es, Morbidität, Mortalität sowie den funktionellen Status nach Re-Bypassoperationen mit und ohne Herz-Lungen-Maschine (HLM) zu evaluieren. Wir haben Morbidität, Mortalität und den funktionellen Status bei 53 elektiven Patienten, die sich zwischen 1998 und 2000 an unserer Abteilung einer erneuten Bypassoperation unterzogen haben, verglichen. 24 Patienten erhielten die Re-Bypassoperation mit und 29 Patienten erhielten die Re-Bypassoperation ohne Herz-Lungen-Maschine. Das Follow-up wurde definiert durch die Freiheit von Angina pectoris (Canadian Cardiovascular Society angina class [CCS class]), Freiheit von erneuten Hospitalisierungen und Reinterventionen sowie in Abhängigkeit von antianginöser Medikation. Das Follow-up war bei allen Patienten vollständig. In der Gruppe, die mit HLM operiert wurde, sind perioperativ 3, in der Gruppe, die ohne HLM operiert wurde, 1 Patient verstorben (n.s.). In der Gruppe mit HLM sind 83 % aller Patienten komplett revaskularisiert worden, wohingegen es in der Gruppe ohne HLM lediglich 14 % waren (p = 0,00001). Die durchschnittliche Nachbeobachtungszeit betrug bei Patienten mit HLM 19 und bei Patienten ohne HLM 14 Monate (n.s.). Kein Patient ist während des Nachbeobachtungszeitraums verstorben. Die durchschnittliche CCS class betrug am Ende der Nachbeobachtungszeit bei Patienten mit HLM 1,4 ± 0,8 und bei Patienten ohne HLM 1,6 ± 0,8 (n.s.). Bei einem Patienten, der ohne HLM operiert wurde, mußte 6 Monate postoperativ ein hochgradig stenosierter Venenbypass zur rechten Kranzarterie dilatiert werden. Patienten ohne HLM hatten am Ende des Nachbeobachtungszeitraums eine höhere Nitratmedikation als Patienten mit HLM (p = 0,013). Das Konzept der Zielgefäßrevaskularisation bei Re-Bypassoperationen ohne Herz-Lungen-Maschine brachte in unserer Patientenserie keinen klinischen Vorteil. Die Zahl an komplett revaskularisierten Patienten war niedriger und die Nitratmedikation im Follow-up höher. Die Re-Bypassoperation ohne HLM ist daher kritisch zu sehen und sollte in erster Linie denjenigen Patienten als Therapieoption vorbehalten bleiben, für die eine komplette Revaskularisation mit HLM mit einem hohen Operationsrisiko verbunden ist.
