Excellent Interobserver Agreement Between a Web-Based Self-Report Questionnaire and Medical Chart Review for the Management of Inflammatory Bowel Disease

Abstract
Introduction: Web-based surveys of patients with inflammatory bowel disease (IBD) are commonly being used to assess health outcomes, patient preferences, delivery of care, health resource use and patient education. However, validating web-based responses from patients is critical to ensure accurate data collection. The objective of the study was to determine if patients with IBD can reliably report clinical data on medications and surgery using a web-based platform. Methods: Patients with Crohn's disease and ulcerative colitis were recruited from Cedars-Sinai Medical Center (n=29), Dartmouth-Hitchcock Medical Center (n=42), The Mayo Clinic Rochester (n=17) and UCLA Medical Center (n=29). Each patient completed a self-report web-based survey that collected information on the following: age at diagnosis; current and past use of azathioprine; current and past use of anti-TNF therapy including certolizumab, adalimumab, and infliximab; and a past history of intestinal resection for Crohn's disease or ulcerative colitis. Research coordinators who were blinded to the survey response independently reviewed the medical chart to record the same information. Inter-observer agreement between self-report survey and chart review were analysed using a kappa statistics with 95% confidence intervals. Kappa >0.80 were considered excellent agreement. Results: The median age of diagnosis was 32 years (interquartile range: 26.5-43.0) and 34.0 years (IQR: 26.0-50.0) for Crohn's disease and ulcerative colitis, respectively. Most responders with Crohn's disease (68.3%) and ulcerative colitis (56%) were female. Based on the chart review 48.3% and 14.0% had a prior history of surgery for Crohn's disease and ulcerative colitis, respectively. Thus, self-report webbased surveys are valid in assessing medical and surgical management of IBD.
