Abstract
The Upper Limb Tremor Reduction in Essential Tremor (ULTRE) study evaluated the safety, tolerability, and efficacy of a wrist-worn vibratory stimulation device in Essential Tremor (ET).Using a double-blind, randomized, sham-controlled, crossover design, two stimulation paradigms were tested: Pulsed (low-amplitude stimulation at 0.125Hz) and Continuous (high-amplitude stimulation at tremor frequency), with comparison to an inactive sham.Patients were required to have a tremor severity ≥2 on one Essential Tremor Rating Assessment Scale (TETRAS items 4, 6, and 8) task and ≥3 on one Bain & Findley Activities of Daily Living (BF-ADL items 2, 4, 17, and 22) task.Assessment tasks were performed before (Baseline), during, and after stimulation.Tolerability and adverse events (AEs) were primary endpoints, while secondary endpoints included improvements in composite BF-ADL (4 Tasks; scores: 4-16) and TETRAS (5 Tasks; scores: 0-20) scores.AEs occurred in 5.6% of participants, all mild-to-moderate and resolving without intervention, with 94% rating the device as comfortable.Statistically significant (p<0.01)improvements from Baseline were seen in both stimulation paradigms: BF-ADL (Pulsed: 1.93±0.56,Continuous: 1.32±0.57)and TETRAS (Pulsed: 1.40±0.60,Continuous: 0.62±0.56).Pulsed stimulation significantly outperformed Sham in both BF-ADL and TETRAS scores (p<0.01), while Continuous stimulation improved BF-ADL (p<0.05).These results demonstrate the safety and efficacy of the Encora Device in reducing tremor.Both stimulation patterns produced statistically significant improvements compared to Baseline and Sham, with Pulsed stimulation showing particularly promising outcomes, indicating that further research is needed to optimize the stimulation paradigm.
