Chemotherapy use in triplet approach for metastatic hormone-sensitive prostate cancer: French practice survey and expert opinion

Abstract
Introduction: Triplet therapy, combining androgen deprivation therapy (ADT) with docetaxel and either darolutamide or abiraterone/prednisone, has significantly improved outcomes in metastatic hormone-sensitive prostate cancer (mHSPC), especially in high-volume disease. However, the absence of standardized eligibility criteria for chemotherapy in current guidelines brings a challenge, potentially leading to underuse in appropriate patients. This study aimed to assess real-world clinical practices in France, evaluating the criteria guiding triplet therapy and leading to docetaxel chemotherapy exclusion. Material and Methods: A national online survey was conducted between August 2023 and February 2024 among French urologists, radiation oncologists, and medical oncologists. Developed by a Steering Committee of 11 French experts, the questionnaire explored chemotherapy decision-making in mHSPC management. Responses were analyzed descriptively. Results: A total of 128 clinicians participated (59% urologists, 20% radiation oncologists, 21% medical oncologists). The primary criteria supporting triplet therapy included high metastatic burden (83%), high-volume disease (80%), and visceral metastases (77%). Conversely, chemotherapy exclusion was frequently based on ECOG Performance Status ≥2 (87%), significant comorbidities (76%), advanced age (≥ 75) (65%), and a G8 score ≤14 (51%). Responses varied across specialties. Discussion/Conclusion: This study highlights the need for standardized criteria to define which patients are considered fit for chemotherapy within a triplet therapy. As its use expands, it is crucial to establish both the disease profiles that justify treatment and the patient-related factors that may warrant withholding it. Integrating findings data with expert opinion can support equitable treatment decisions, avoid loss of opportunity, and ensure that the greatest number of eligible mHSPC patients can benefit from triplet therapy. Cependant, l’absence de critères d’éligibilité standardisés pour la chimiothérapie dans les recommandations actuelles pose un défi, pouvant conduire à une sous-utilisation chez les patients appropriés. Cette étude visait à évaluer les pratiques cliniques en situation réelle en France, en examinant les critères guidant la thérapie par triplet et les facteurs menant à l’exclusion de la chimiothérapie par docétaxel. Matériel et Méthodes : Une enquête nationale en ligne a été menée entre août 2023 et février 2024 auprès d’urologues, de radio-oncologues et d’oncologues médicaux français. Élaboré par un comité de pilotage composé de 11 experts français, le questionnaire a exploré la prise de décision concernant la chimiothérapie dans la prise en charge du mHSPC. Les réponses ont été analysées de manière descriptive. Résultats : Au total, 128 cliniciens ont participé (59 % d’urologues, 20 % de radio-oncologues, 21 % d’oncologues médicaux). Les principaux critères justifiant la thérapie par triplet incluaient une charge métastatique élevée (83 %), une maladie à volume élevé (80 %) et des métastases viscérales (77 %). À l’inverse, la non-recours à la chimiothérapie était fréquemment fondé sur un statut de performance ECOG ≥2 (87 %), des comorbidités significatives (76 %), un âge avancé (≥ 75 ans) (65 %) et un score G8 ≤14 (51 %). Les réponses variaient selon les spécialités. Discussion/Conclusion : Cette étude met en évidence la nécessité de critères standardisés pour définir quels patients sont éligibles à la chimiothérapie dans le cadre d’une thérapie par triplet. Alors que son utilisation s’étend, il est crucial d’établir à la fois les profils de maladie justifiant le traitement et les facteurs liés au patient pouvant motiver son exclusion. L’intégration des données issues de cette enquête avec l’avis d’experts peut soutenir des décisions thérapeutiques équitables, éviter les pertes de chances et garantir que le plus grand nombre de patients éligibles atteints de mHSPC puisse bénéficier de la thérapie par triplet.
