Mepolizumab reduces exacerbations and improves quality of life in COPD irrespective of severe exacerbation history: Pooled Phase III trial results

Abstract
Background: METREX/METREO/MATINEE recruited patients with COPD and prior moderate/severe exacerbations. Aims: Assess mepolizumab in Phase III METREX/METREO/MATINEE trials ( NCT02105948 /02105961/04133909) in patients with type 2 inflammation indicated by raised blood eosinophil count (BEC) with/without prior severe exacerbation(s). Methods: Post hoc analysis of pooled data from patients with BEC ≥300 cells/µL (N=1146; 568 mepolizumab), with/without prior severe exacerbation(s) (≥1/0 exacerbation[s] needing ≥24-hour hospitalisation in 12 months prior to screening). We report annualised rate of moderate/severe exacerbations, and Week 52 St. George’s Respiratory Questionnaire (SGRQ) and COPD Assessment Test (CAT) responders (≥4-point and ≥2-unit reduction, respectively). Results: See Table. In 288 (25%) patients with prior severe exacerbation(s), the annualised rate of moderate/severe exacerbations was 24% lower with mepolizumab vs PBO; 22% lower in those without (rate ratio [95% CI]: 0.76 [0.58, 0.98] and 0.78 [0.66, 0.94], respectively). The odds of SGRQ response were higher with mepolizumab vs PBO in both subgroups. No significant improvement seen with mepolizumab over PBO in CAT responders. Conclusions: Mepolizumab significantly improved exacerbations and SGRQ in patients with COPD, irrespective of severe exacerbation history.
