Wirksamkeit und Sicherheit einer Guselkumab Induktionstherapie bei Patient:innen mit mittelschwerer bis schwerer aktiver Colitis ulcerosa: Ergebnisse der Phase-2b-Studie QUASAR über 12 Wochen

Abstract
Einleitung und Ziele Die QUASAR-Induktionsstudie ist eine randomisierte, doppelblinde, Placebo-kontrollierte Phase-2b-Dosisfindungsstudie. Im Rahmen der Studie wird die Wirksamkeit und Verträglichkeit des Interleukin-23-Inhibitors Guselkumab bei Patient:innen mit mittelschwerer bis schwerer aktiver Colitis ulcerosa untersucht. Die Studienteilnehmer:innen wiesen zuvor ein unzureichendes Ansprechen oder Unverträglichkeit auf konventionelle Therapien und/ oder Biologika auf. Methodik In die Auswertung eingeschlossen waren Patient:innen mit mittelschwerer bis schwerer aktiver Colitis ulcerosa, definiert als modifizierter Mayo-Score von 5-9,≥ 1 Punkt im Subscore „rektale Blutung“ und≥ 2 Punkten zur Baseline im Endoskopie-Subscore. Die Studienteilnehmer:innen wurden 1:1:1 randomisiert auf Guselkumab 200 mg i. v., 400 mg i. v. oder Placebo zu Woche 0, 4 und 8. Primärer Endpunkt war das klinische Ansprechen zu Woche 12. Als wichtige sekundäre Endpunkte waren klinische Remission, symptomatische Remission, endoskopische Verbesserung, histo-endoskopische mukosale Verbesserung und endoskopische Normalisierung zu Woche 12 definiert. Das Sicherheitsprofil von Guselkumab wurde bis Woche 12 ausgewertet. Ergebnis Es wurden die Ergebnisse von 313 Patient:innen ausgewertet; demographische Daten zur Baseline und Krankheitsverläufe waren zwischen den Behandlungsgruppen vergleichbar ([ Tabelle 1 ]). Zu Woche 12 erreichten unter Guselkumab 200 mg i.v. und 400 mg i.v. signifikant größere Anteile der Patient:innen ein klinisches Ansprechen als unter Placebo (61,4% und 60,7% vs. 27.6%; p<0,001). Zudem erreichten unter Guselkumab im Vergleich zu Placebo mehr Patient:innen zu Woche 12 wichtige sekundäre Endpunkte ([ Abb. 1 ]). Die Anteile der Patient:innen mit unerwünschten Ereignissen, schweren unerwünschten Ereignissen und unerwünschten Ergebnissen, die zu einem Therapieabbruch führten, waren unter beiden Guselkumab-Dosierungen nicht höher als unter Placebo ([ Tabelle 2 ]). Schwerwiegende Infektionen, Malignome oder Todesfälle wurden unter Guselkumab nicht beobachtet.
