Abstract
目的:電気的胎児心拍数モニタリング(Electronic Petal Monitoring:EFM)は世界各地で多くの医療\n機関に普及し、日本においてもほとんどの産科施設に備えられ、ルチーンに用いられている場合も多い。し\nかし分娩管理における有効性と安全性については、偽陽性率が高いこと、EFM使用により機械的分娩率が\n上昇すること等の問題点が指摘されている。そこでEFM に関する内容を含む分娩管理に関する各種ガイド\nラインの質評価を行い、低リスク妊娠の分娩管理において、EFM がどのような位置付けにあるかを比較検\n討することにより、低リスク妊娠での今日におけるEvidence-basedのEFM適用基準・使用方法を明らか\nにすることを目的とした。\n方法:EFM適用に関する内容を含む分娩管理に関するガイドラインを、データベースのエレクトロニッ\nク・サーチ及びこれらの検索で得られたガイドラインのReferencesチェックにより収集した。得られたガ\nイドラインについて、2種のガイドライン採点方法を用いた採点と、ガイドラインのエビデンス比較により\n質評価を行った。\n結果:採点結果及び勧告内容とエビデンスの比較結果による総合評価から、Evidence-basedのガイドラ\nインとしてRoyal College of Obstetricians and Gynaecologists(RCOG)によるガイドラインが最も質\nが高く、次にSociety of Obstetricians and Gynaecologists of Canada(SOGC)によるものが優れたガイ\nドラインであった。これらのガイドラインでは低リスク分娩でのEFM のルチーンな使用は推奨されておら\nず、間歌的聴診法(Intermittent Auscultation:IA)を推奨していた。\n結論:低リスク妊娠における分娩時胎児モニタリングに関する第一選択は、入院時スクリーニング・分娩\n中胎児監視いずれの場合でも間歌的聴診法であり、分娩時におけるEFM の適用は、母体・胎児のリスク評\n価により適切に症例を選択して行われるべきである。
